A New Combination Success in Advanced Lung Cancer: Zipalertinib Plus Chemotherapy for EGFR Exon 20 Insertions
In the Phase 3 REZILIENT3 trial, adding zipalertinib to chemotherapy halved the risk of disease progression or death in previously untreated patients with advanced non-small cell lung cancer harboring EGFR exon 20 insertion mutations. Median progression-free survival increased from 8.5 to 14.5 months.
13 September 2026 — Data from the Phase 3 REZILIENT3 trial, presented at the International Association for the Study of Lung Cancer (IASLC) 2026 World Conference on Lung Cancer (WCLC), point to a clinically meaningful improvement in the first-line treatment of patients with advanced non-small cell lung cancer (NSCLC) harboring EGFR exon 20 insertion mutations.
Adding the targeted agent zipalertinib to platinum-pemetrexed chemotherapy halved the risk of disease progression compared with chemotherapy alone. The result once again demonstrates the strength of rational combinations in overcoming cellular resistance.
The REZILIENT3 Trial and Improved Survival Data
The key efficacy results of REZILIENT3 (NCT05973773), an international, randomized, open-label Phase 3 trial that enrolled 279 patients with EGFR exon 20 insertion mutations who had not previously been treated for advanced disease, are as follows:
- Progression-Free Survival (PFS): Median progression-free survival, as assessed by independent central review, was 14.5 months in the zipalertinib plus chemotherapy arm versus 8.5 months in the chemotherapy-alone arm (hazard ratio, HR: 0.50). This corresponds to a 50% reduction in the risk of disease progression or death.
- Efficacy in Brain Metastases: The PFS benefit of the combination was also consistently observed in the subgroup of patients with asymptomatic, untreated brain metastases, one of the most difficult situations to treat in lung cancer (HR: 0.38).
- Objective Response Rate (ORR): The objective response rate reached 65.0% in the combination arm, compared with 40.3% in the control arm.
- Duration of Response (DOR): Among patients who responded to treatment, the median duration of response was 14.2 months in the combination arm and 9.9 months in the control arm.
- Overall Survival (OS): At an interim overall survival analysis with 30% maturity, a favorable trend in favor of the combination was reported (HR: 0.72).
Side Effect Profile and Clinical Management
The side effect profile of the combination was consistent with the known safety profiles of the agents used (zipalertinib and platinum-pemetrexed).
- Grade 3 or higher adverse events: Grade 3 or higher adverse events in patients receiving the zipalertinib combination (87.1%) were driven mainly by hematologic (blood count) changes that are well manageable in the clinical setting.
- Toxicities related to EGFR inhibition: Grade 3 or higher toxicities related to targeted EGFR inhibition remained low, with rash reported in 10.7% and diarrhea in 1.4% of patients.

Innovative treatment approaches such as the zipalertinib and chemotherapy combination, whose efficacy data are shared in this article, have been presented at international scientific meetings; the official licensing, conditions of access and Social Security Institution (SGK) reimbursement status of such next-generation targeted agents in Turkey may vary. Conclusion
REZILIENT3 showed that adding zipalertinib to first-line chemotherapy in advanced lung cancer with EGFR exon 20 insertion mutations extends progression-free survival by about six months, and that this benefit is maintained in patients with brain metastases. Overall survival data are not yet mature; the full picture will become clear with further analyses and regulatory review. Detecting this rare mutation at diagnosis remains decisive for patients to benefit from targeted options of this kind.
